Out of Hours
If you have an urgent need to see a doctor when the surgery is closed, telephone 03336 664664 to contact Northampton Out of Hours. Only use this service if your problem cannot wait until the surgery is open. Remember that the out of hours doctor does not have access to your complete medical record.

Appointments
To book an appointment telephone 01604 644126. You may book with any doctor, not only the one with whom you are registered. If you are unable to keep your appointment, please cancel it by telephoning the surgery so another patient may use the appointment.

Registration

registrationWe are happy to register new patients living in our practice area. To register with us you will need to bring your medical card with you or alternatively you will need to fill in a registration form. An online version of the registration form is on the pre-registration page.

You will also be asked to complete a new patient questionnaire and if you are in our dispensing area you will need to complete a form for this too. You will also be asked for 2 forms of identification one of which should be photo ID such as a passport or driving licence. The other should show the address at which you are living in our practice area.

If you are from overseas you will be asked to show your passport and visa.

You may be asked to attend an appointment with a nurse if you have a long term condition or if you take regular medication.
